RFA forms 12/2012 vs 07/2013 (California)

I have been using form RFA9785.5, but this week I received a missive from Berkshire Hathaway that we have to use a new form: DWC Form RFA (ver 07/2013) which apparently became effective as of July 1. We have never been notified of the introduction of a new RFA form, and I cannot find this new form on the DWC website. I can find only the RFA9785.5 (12/2012).

Can someone please direct me to the new Form RFA (ver 07/2013)?

I would also appreciate clarification of the adjusters’ responsibilities with regard to the RFA forms. We submit the completed forms to the adjuster and to UR. When we get medical certification, we VERY RARELY get a signed authorization from the adjuster. Are the adjusters required to sign the authorization form and return it to us if a procedure is authorized?

I haven't seen it. I don't know how they can demand you use a form that is not published yet...

Tell them to fax you a copy!

If they don't sign it, how do you know it is authorized?
