What fee schedule do I use for determining the proper reimbursement of compound medications?

The link above will take you to the DWC website were a prescription calculator is provided as i call it. Its going to ask you, how many ingredients there are in the compound. In order to actually price compound meds your going to need a "compound breakdown sheet", essentially a breakdown of the compounded medication ie grams, unit price, NDC#. In most cases the opposing side will request a copy of the breakdown to justify reimbursement.
