I work for a small Defense firm.

Any suggestion on a EAMS/ Case Managment forms package?

I'm not sure what type of forms you are referring to, but check out this site. It has all of the EAMS/OCR forms for free:

What do you want to do with an EAMS/Case Management system?

ScanFiles has PerfectFiler. This is available only to their customers. DWC Direct is working on a web based solution but it is in testing phase only right now.

A forms package which saves data on a file . Then it can auto populate forms on the same file.

The DWC web site forms is what we are using now. Seems kind of slow to go to the site and type in the data each time on each file. Although being free the price is right as noted above.

We use Abacus Law. It's a great program. License's are a bit costly but it's well worth it in time and convenience.

Abacus offers case management and has a specific California Workers Compensation module and has many other features.

We use it for pre-populated forms, accounting, calendaring.

They have pre-populated forms for comp. You can also create your own forms, letters, pleadings, POS. It's fairly easy to learn how to pre-populate your own forms.
